Do you accept my insurance?
I am considered Out-Of-Network (OON) with all insurance providers. This allows me to have more autonomy in your care and decide what treatment plan is the best option for you. A superbill to submit to your insurance company for possible reimbursement can be provided upon request and may go toward your OON benefits for occupational therapy services. It is the clients responsibility to check benefits regarding reimbursement.
Do you have a cancellation policy?
Yes, we require 24 hours notice for cancellation without penalty. Cancellations made less than 24 hours prior to an appointment may result in a fee. Appointments that are not cancelled are subject to full charge for the visit.
Can I set up a payment plan?
Yes.
Do I need a physician referral?
Yes. In the state of Illinois it's required to have a physician referral for occupational therapy services even if you are not submitting a superbill.

What is your covid policy?We are following all state and federal regulations, and are frequently updating our practices to reflect the most up to date information available from the CDC and other organizations. Please contact us with questions concerning our current policies in order to keep our patients and providers safe.

Where are you located?All in-person sessions take place at Restore Pilates located at 4845 N Damen Ave, Chicago, IL, 60625. Street parking is available. There is a doorbell to notify your provider that you have arrived.
