Answers to frequently asked questions about the wellness program.

Do you accept my insurance?

I am considered Out-Of-Network (OON) with all insurance providers. This allows me to have more autonomy in your care and decide what treatment plan is the best option for you.  A superbill to submit to your insurance company for possible reimbursement can be provided upon request and may go toward your OON benefits for occupational therapy services. It is the clients responsibility to check benefits regarding reimbursement.

Do you have a cancellation policy?

Yes, we require 24 hours notice for cancellation without penalty. Cancellations made less than 24 hours prior to an appointment may result in a fee. Appointments that are not cancelled are subject to full charge for the visit.

Can I set up a payment plan?

Yes.

Do I need a physician referral?

Yes. In the state of Illinois it's required to have a physician referral for occupational therapy services even if you are not submitting a superbill. 

When is payment required?

A credit card is required to reserve your appointment and payment is required at completion of the appointment.
